Statement of Consent/Medical Release:</b><br>In the event of an emergency or non-emergency situation requiring medical treatment, I, the purchaser of this class, hereby grant permission for any and all medical and/or dental attention to be administered to my child/children, in the event of an accidental injury or illness, until such a time as I can be contacted. This permission includes, but is not limited to, the administration of first aid, the use of an ambulance, and the administration of anesthesia and/or surgery, under the recommendation of a qualified medical personnel. I understand that every precaution is taken to secure the safety of each participant; however in the case of an accident, I agree to release Hands on Pottery from any and all liabilities.<br><br><b>3. Behavior Consent:</b><br>If your child chooses not to behave properly or listen to camp staff we have the right to call the child's parents to have the child picked up immediately. After parents have been contacted, they can return the next day. If the child again chooses not to behave the parents will be called. The child will be permanently removed from the camp and no refunds will be given.<br><br><b>4. Photo Release:</b><br>During camp activities, photos and/or videos will be taken. They may be used in publicity for the Kids Camp or Hand on Pottery. Not all children are photographed, and those who are photographed, not all will necessarily be featured in print or online. Please indicate whether or not your child has permission to be included in the photos taken of camp activities. Of course this is not a guarantee that you child's photo will ever be publicized. Please complete this part under the questions section of the booking process to indicate if your can may or may not be photographed during camp activities and whether those photos may be use in the Kids camp and/or Hands on Pottery's publicity materials.</div></div>
